🧩 Adding Items to Your Questionnaire

Questionnaire items are what bring your questionnaire to life. Items allow you to ask questions, add helpful context, and organise information so clients can respond easily and confidently.

Every questionnaire is built by adding items one at a time.

✨ Where to Add Items

When editing a questionnaire, scroll to the Items section and click Add Item +. This opens the item menu, where you can choose exactly what to add next.


🧾 Content Items

Content items help guide client through your questionnaire by providing context, instructions, or visual breaks. They don’t require a response.

📝 Text Block

Use a text block to add explanations, instructions, or helpful notes for your clients This is ideal for introducing a section or clarifying what information you’re asking for.

Use a Text Block to:

  • Add instructions

  • Explain what the next section is about

  • Provide additional details to your clients

🖼️ Image / Photo

Add an image or photo to provide visual context or inspiration. This can help set the tone or support the information you’re requesting.

Use this block to:

  • Add visual references

  • Upload inspiration images

  • Share examples or mood boards

📄 Page Break

Use a page break to split longer questionnaires into clear sections. This keeps your questionnaire organised and easier for clients to complete.

Page Breaks help you:

  • Split the Questionnaire into sections

  • Make long forms easier to complete

  • Improve the overall user experience


❓ Question Items

Question items allow you to collect responses from your clients. Choose the question type based on the kind of answer you need.

✏️ Short Text

Best for:

  • Names

  • Short answers

  • Email addresses or quick notes

📝 Long Text

Best for:

  • Detailed explanations

  • Open-ended answers

  • Notes or special requests

📋 Dropdown

Best for:

  • Long lists of options

  • When only one option should be selected

  • Keeping the form clean and compact

🔘 Choose One

Best for:

  • Single-choice questions

  • Clear yes/no–style decisions with multiple options

☑️ Multiple Choice

Best for:

  • Selecting more than one option

  • Preferences with multiple answers

📅 Date Select

Best for:

  • Event dates

  • Deadlines

  • Preferred timelines

✅ Yes / No

Best for:

  • Simple confirmations

  • Binary decisions


Note: Only the planner can create these questions, and they are visible exclusively within the planner dashboard.
